TEAM REGISTRATION FORM

Saturday and Sunday, June 25 & 26 (each team plays one day only) Tualatin Hills Parks and Rec Softball Fields 158th and Walker Beaverton, OR

General Team Information:

With the price of a team, you will receive:

1. Lunch for 14 players
2. Commemorative t-shirts for 14 players
3. Guaranteed three games (teams will only play either Saturday or Sunday)
4. Digital team photo
5. A day of fun for employees and their families

Things to remember:

• For men's competitive, you will need a minumum of 10 men.
• For co-ed recreational, you will need an even number of men and women, minumum of 10. If you have more men than women, then when two men bat in a row, there is an automatic out charged to the team.
• Additional t-shirts and lunches will be on sale for other players and family members on the day of the event.
